PALS—Patient Advocacy & Liaison Service

PALS is Allen Home Care Services’ Patient Advocacy and Liaison Services program. Revolutionary in its scope, PALS offers true peace of mind to the patient and their family by insuring that the communication flow between patient and physician is thorough and accurate. PALS is the link between the patient and all other health care providers. Upon enrollment, a Registered Nurse is dispatched to the home (or other location) to make an initial assessment.

The initial assessment includes:

Catalogue all current and previously prescribed medications as well as durable medical equipment used by the patient

Schedule existing appointments with other health care professionals

Determine the overall physical condition and capabilities of the patient

Build a knowledge base of physician enrollment, i.e. primary care doctor and specialty doctor(s)

Build a knowledge base of the patient’s family contacts so that lines of communication can be developed for initial and follow up care

A thorough orientation on the patient’s medical history and lifestyle as needed

Follow up is available depending upon the customized needs of the patient. Allen Home Care Services can provide for a multiple of services selected ala cart from a menu that can be adjusted throughout the enrollment period.

Follow up services can include:

Attend doctor visits with the patient so that complete information is conveyed to the doctor in an effort to insure the best treatment plan is developed by the physician

Review with the patient in laymen’s terms on the prescribed treatment plans and expectations of progress

Pick up prescribed medications for the patient

Assist patient to understand the purpose, accurate dosing and unwanted side effects of medications

Make recommendations on diet and nutrition

Arrange for transportation to the doctor’s office

Conduct regularly scheduled follow up visits in the patient’s home (or other facility) as often as selected by the patient and their family

Conduct “phone friends” calls on a regular basis to stay in touch with the patient

Report on the patient’s progress to designated family members

This program is ideal for elderly and chronically ill patients whose local family can not take off from work in order to attend doctor’s visits. It is also aimed for those patients who do not have family living in the immediate area. PALS provides true peace of mind to the patient and their family by insuring that their medical progress is being implemented by someone who knows them as well as they know themselves.

The patient and their family can rest assured that all information is kept strictly confidential.

Since this program is on the “cutting edge” of health care, most insurance companies and other third party payers do not provide for this kind of service. Just because it is not covered, however, does not mean it is not needed! Allen Home Care Services can arrange for private payment by various means, including accepting most major credit cards.
